First Grant Cycle - 
The CFLF has currently suspended our March 31, 2020 Recreation Grant cycle due to COVID-19 impacts on CFLF fundraising and closures of most recreational activities. It is our hope that in the near future with financial stabilization we will be able to establish a temporary program to provide exercise related assistance in response to the needs we hear from the CF Community. Applications that have already been received will be considered for alternate assistance at the time.

Frequently Asked Questions:


Q: Can I apply for more than one activity?

A: Yes, as long as the total of ALL activities is $500 or less you may apply for multiple activities.  Requests made that are over $500 (for an individual grant) will only be funded up to the $500 maximum.

 

Q: How do I know if I (our family) is financially eligible?

A: Eligibility is based on being at or below 400% of Federal Poverty Level for individuals with CF or families with one child with CF. Families with 2 or more children with CF are considered eligible at or below 500% of the Federal Poverty Level. To calculate your eligibility, find your family size HERE and mulitiply the income level by 4 (or mulitply by 5 if there is more than one person in your household with CF). If your household income is at or below that number, you ARE eligible for a CFLF Recreation Grant.

 

Q: How and when will I (or my child) hear a decision from CFLF regarding a decision on the requested Recreation Grant?

A: Applicants will be emailed with either an approval or denial approximately one month after the deadline. Based on the number of applications received it could be a week or two later. If you have not heard a decision about your grant request by 5/15 (or 10/15 for the second grant cycle) you may email Program Coordinator Erin Evans at Erin@CFLF.org

 

Q: What if I don’t have a photo of myself doing my chosen activity?

A: We prefer the photo to be of the activity the person is applying for. However, if one is unavailable you may submit a photo of yourself doing a different activity.  Please do not send photos of just head shots, or non-active poses.  All photos must be of ONLY the individual, due to privacy issues.

 

Q: Can I apply for a grant to get reimbursed for an activity that has already been paid for?

A: No, unfortunately requests made for reimbursement will not be considered.

 

Q: If my child is too young to write or understand the application questions, can I answer them for him/her?

A: Yes, if your child is unable or too young and does not understand the questions, their parent or guardian may help in filling out the applications on their behalf, from the parent/guardians perspective.  It is preferred that as much of their own words are used in the application, so using the questions to ‘interview’ them is often a great option.

 

Q: Can I use my grant to register for a fundraising event for another organization?

A: The only fundraising events or participation that the CFLF Recreation Grants would be eligible to support would be for CFLF.  

 

Q: Can I apply for a grant to purchase a bike, treadmill, or other equipment?

A: Typically grants are only for activity costs (i.e., gym memberships, summer or sports camps, exercise classes, swim lessons, martial arts, etc.), and not for equipment purchases.  If you feel that you have an extenuating circumstance, which would be eligible for the CFLF to consider an exception, you may contact Grants@cflf.org to inquire or submit an application that thoroughly explains your situation, with as much supporting evidence as possible.  Exceptions are not guaranteed.  

 

Q: What should I do if I haven’t heard anything from CFLF and am wondering about the status of my grant?

A: You may e-mail Program Coordinator, Erin Evans, at Erin@cflf.org and inquire about the applicants status.  Make sure to include the applicant’s name and date of birth.

 

Q: How often can I apply for a recreation grant from CFLF?

A: Grant recipients may re-apply every year for the same or a different activity.  Individuals are eligible to be awarded one grant per 12-months which is determined by the date of approval.  For example if someone is approved on June 1, 2017, then they will be eligible to be approved again on June 1, 2018.

 

Q: Can my doctor send in the page they need to fill out (the last pages of the application) on its own, or does it need to be included with the rest of the application?

A: The application should come to us fully complete, which means all pieces of the application (each page filled out and a photo) should be submitted together. Due to high volume of applications we can not guarantee applications that are sent in separately will be saved and kept together.

 

Q: What if my child is too young to provide PFT scores?

A: Have your doctor put a note on the page where we ask for PFT scores that the child is too young for lung function tests

 

Q: Can I apply for a Recreation Grant if I live outside of the US?

A: With such severely limited funds, the CFLF has needed to focus on patient requests from within the US. Our understanding for patients in the UK is that the Cystic Fibrosis Trust also offers a similar program of supporting recreational activity expenses. We are not aware of any others internationally, but would love to know if you happen to find any.
